Hospice - Patient and Family Rights
Patients and their family members have the right to the following from a Medicare-certified hospice provider:
- An MD or DO Medical Director overseeing the patient’s medical treatment.
- Continuity of care whether the patient is at home or in a health care facility.
- An interdisciplinary team working together to develop and implement the patient’s personal plan of care. This group should include a physician, registered nurse, social worker, and counselor.
- Reasonable accommodation allowing visits from a religious advisor of the patient's choice.
- The right to end hospice care and resume curative treatment, and the right to return to hospice if the patient is qualified.
- The right to receive the highest quality care available.
- The right to be informed of a change in the plan of care, what the change means, and the consequences if the patient refuses the new plan of care.
- The right to know how to file a complaint and how to transfer to another hospice.
- The right to be notified in writing of patient rights before services or treatment is provided.
- The right to not be discriminated against based on age, race, color, sex, gender, religion, or disability.
- The right to sign or to refuse to sign an Advance Directive and a Do-Not Resuscitate Order.
